The government has made it compulsory to provide health information in the uniform admission form for all primary schools.
An office order issued on Tuesday by Bhupesh Ranjan Roy, assistant director (Policy and Operation) of the Directorate of Primary Education, confirmed the directive.
According to the order, a standardized admission form has been developed to include necessary health‑related information for every student enrolling in primary schools nationwide.
Authorities have been instructed to record health information of current students for the ongoing academic year using the uniform form, and to ensure the admission process is implemented through the same form from the next academic year.
