A groundbreaking study has revealed that psilocybin, the active compound in magic mushrooms, is significantly more effective than traditional nicotine patches for individuals attempting to quit smoking.

The research, published in March 2026, suggests that a single dose of the psychedelic combined with talk therapy can facilitate a profound shift in worldview, helping users abandon cigarettes.

Significant success rates

The clinical trial, led by Matthew Johnson, a professor of psychiatry and behavioural science at Johns Hopkins University, involved 82 participants.

Subjects were randomly assigned to receive either a high dose of psilocybin or a nicotine patch, with both groups participating in 10 sessions of cognitive behavioural therapy, reports BBC.

Six months after treatment, 52 percent of the psilocybin group remained abstinent, compared to only 25 percent in the nicotine patch group. These findings echo a smaller 2014 pilot study where 80 percent of participants were still smoke-free six months after receiving psilocybin-assisted therapy.

Professor Johnson noted that the “magnitude of the experience” often outweighed the psychological barriers of addiction.

Addressing a global health challenge

Nicotine is considered as addictive as heroin or cocaine, making tobacco one of the most difficult substances to quit.

Lynn Marie Morski, executive director of the Psychedelic Medicine Association, described the study as one of the most impactful lines of inquiry in psychedelic science regarding the reduction of preventable deaths. Megan Piper, a professor at the University of Wisconsin, Madison, added that because no new smoking cessation medications have emerged in two decades, the potential of this treatment is “exciting”.

Neurological “windows of opportunity”

Researchers are also uncovering why these drugs are effective. Gül Dölen, a neuroscientist at the University of California, Berkeley, explained that psychedelics can reopen “critical periods” in the brain. These are windows of heightened malleability, typically seen in childhood, which allow for the unlearning of old habits and the adoption of new ones through therapy.

Professor Johnson further suggested that “behavioural plasticity” allows those stuck in long-term patterns to find a “potential way out”.

Historical funding and outlook

The significance of this research is reflected in a $4 million (£2.9 million) grant from the National Institutes of Health (NIH) in the US for a larger, multi-site study. This marks the first US government grant in over 50 years dedicated to investigating the therapeutic benefits of a psychedelic drug.

However, experts urge a measured approach. Professor Piper cautioned that the initial trials involved a small, mostly white, and highly educated demographic, and it remains unclear if psilocybin would be equally effective across more diverse populations.

Furthermore, psilocybin remains illegal in most countries, and scientists continue to monitor for any long-term risks or side effects that have yet to be identified.

Dominique Morisano, a clinical psychologist at the University of Toronto, suggested that the benefits could be further bolstered by integrating mindfulness and exercise into the treatment programme to help maintain long-term recovery.
